Brittle Bone Disease Information

Brittle Bone Diseases (Osteogenesis Imperfecta)

The brittle bone diseases are a large group of conditions characterised by excessive fragility of
the bones, present from childhood.
The most common cause of the disorder is osteogenesis imperfecta which is a group of
conditions all caused by inherited defects of collagen (the protein responsible for the strength of
the bone). The severity of the condition may vary. Some affected individuals have multiple
fractures, in others the condition is difficult to diagnose.
It is conventional to describe four varieties of osteogenesis imperfecta. Type II usually causes
such severe fractures that infants are stillborn or die in the first few weeks of life. Type III is
severe but consistent with good survival although there may be markedly impaired growth.
Types I and IV are milder conditions with good stature and relatively few fractures. Type I is
characterised by distinctly blue/grey sclerae (whites of the eyes) which is not seen in Type IV.
Associated problems may include lax joints, fragile teeth, excessive sweating and easy bruising.
Other less common causes of brittle bone disease include osteopetrosis hypophosphatasia,
fibrous dysplasia (Albright Syndrome) and ideopathic juvenile osteoporosis. Additionally
temporary brittle bone disease may cause fractures in the first year of life and improve later.
This is still not well understood. Fracture in childhod may also be caused by rickets due to
vitamin D deficiency.

Brittle Bone Disease Drug Side Effects Information
Zometa and Fosamax ( generic name - Alendronate ) is a type of drug known as
bisphosphonates. Recently a side effect link has been found between bisphosphonates and a
serious brittle bone disease called osteonecrosis of the jaw ( ONJ ). This important discovery
clearly shows that Zometa side effects may include osteonecrosis of the jaw, a serious side
effect.
The discovery, published in the Journal of Oral and Maxillofacial Surgeons, prompted both the US,
Food and Drug Administration ( FDA ) and Novartis, the manufacturer of bisphosphonates used in
cancer chemotherapy and hypercalcemia, to issue a warning to health care professionals on
September 24, 2004. The warning letter contained information about bisphosphonates and the risks
of osteonecrosis in the jaw.
Bisphosphonates are commonly used in tablet form such as Zometa and Fosamax ( Alendronate
Sodium ) to prevent and treat osteoporosis and brittle bone disease in post-menopausal women and
Hypercalcemia
Stronger forms of bisphosphonates are commonly used in the management of advanced cancers
that have metastasized to the bone, where the disease often causes bone pain and possibly even
fractures. Several cancers can involve or metastasize to the bone, including lung cancer, breast
cancer, prostate cancer, multiple myeloma, and others. When bisphosphonates are given in cancer
chemotherapy, the drugs are given intravenously, and usually for longer periods of time.

Osteonecrosis of the Jaw
Osteonecrosis of the jaw ( ONJ ) is a condition in which the bone tissue in the jaw fails to heal after
minor trauma such as a tooth extraction, causing the bone to be exposed. The exposure can
eventually lead to infection and fracture and may require long-term antibiotic therapy or surgery to
remove the dying bone tissue. Experts say that prevention and early treatment of individuals using
bisphosphonates such as Fosamax is extremely important in preserving the jaw bone. Individuals
using Fosomax or other bisphosphonates should attempt to avoid tooth extractions and other major
dental work while on the drugs.

Although most cases of osteonecrosis of the jaw related to Fosamax side effects have occured
after dental work several cases have been reported to occur without any prior dental work. If you are
currently taking Fosamax you may be at risk of developing osteonecrosis. If you have been injured
